CONSENT FOR TELEMEDICINE SERVICES
You’ll be communicating with your provider through two-way audio and video, and not in-person.
In some cases, a remote visit will not be sufficient, and an in-person visit, with our provider or another provider, will be needed for proper evaluation. Your provider will tell you if this is the case.
Technical problems might lead to delays, or make the visit impossible. In this case, you’ll be contacted by phone or some other means.
For security and privacy, your video will utilize end-to-end encryption and no copies of the audio or video visit will be stored. Even with these safeguards in place, security breaches may still be possible.
