What is RISE?

RISE (Reclaim Inner Strength, Evolve) is Conscientia Health’s integrated teen wellness program located on the Red Bank Catholic campus. RISE provides students with confidential, clinician-led support for their emotional, mental, and physical well-being through coordinated Psychiatry, Therapy, and Primary Care services. 

R.I.S.E. stands for:

  • Inner – Focus on your thoughts, emotions, and overall well-being.

Who is RISE for?

RISE is designed for teens who may benefit from mental health, behavioral health, or primary care support. The program works collaboratively with parents or guardians while helping teens build resilience, confidence, and healthy coping skills. 

What services does RISE offer?
  • RISE provides: 

    • Psychiatry for mental health evaluations and medication management 
    • Therapy for emotional support, stress, anxiety, depression, and life challenges 
    • Primary Care for wellness visits, physicals, and general health concerns

RISE brings mental health and primary care together in one coordinated program. Our integrated model allows providers to work collaboratively when appropriate, helping teens receive comprehensive support while giving families peace of mind.

Can my teen receive more than one type of service?

Yes. Teens may participate in Psychiatry, Therapy, Primary Care, or a combination of services based on their individual needs. Our goal is to provide seamless, coordinated care. 

How does my teen get connected to RISE?

Students can connect with RISE by: 

  • Speaking with RBC Counseling or a Teen Assistance Counselor 
  • Visiting the Wellness Room 
  • Scanning the RISE QR code or visiting the program landing page 
  • Calling Conscientia Health directly using the number provided on campus 
How do I schedule an appointment?

Appointments can be scheduled online or by calling 877-803-5342 and selecting Option 4 for RISE. A member of our team will assist with scheduling and onboarding. 

Are virtual appointments available?

Yes. RISE offers both in-person and telehealth appointments, making care accessible and convenient for students and families. 

What are RISE's hours?

RISE is available seven days a week from 7:00 AM to 10:00 PM by appointment. 

Is my teen's information kept confidential?

Yes. All health information is protected under HIPAA and applicable privacy laws. We are committed to providing a trusted environment where teens can openly discuss concerns and receive professional support. 

Will parents or guardians be involved in care?

Yes. Parents and guardians play an important role in their teen’s care. With the appropriate consent and authorization on file, our team works collaboratively with families while respecting privacy and clinical best practices.

How do I request a prescription refill or report medication concerns?

Prescription refill requests can be submitted through the patient portal, by email, or by calling our office. If your teen experiences medication side effects or concerns, please contact us so we can notify the provider and determine the appropriate next steps. 

What should I do if my teen is experiencing a mental health emergency?

If your teen is experiencing an immediate mental health or medical emergency, call 911, contact 988, or go to the nearest emergency room. RISE provides ongoing care and support, but emergencies require immediate intervention through emergency or crisis services.
